How to Change the Name & Date of Birth (DOB) on a Birth Certificate

A birth certificate is one of the most important document in a person’s life. It is usually used as the proof of birth and date of birth. A lot of government organizations grant it as a proof. So, if there is a mistake in the certificate, it may cause some serious problems to the person whom it belongs. But you don’t have to worry as the details can be changed. Here we are giving information on both online and offline process about changing the name or date of birth on a birth certificate. Both the processes are described below.

How to Change the Name & Date of Birth (DOB) on a Birth Certificate

Online Process:

  • First of all, you need to visit the official birth enrolment website of your state.
  • You will get there the option of birth and death corrections. Click on that.
  • Fill the form properly along with the changes.
  • Upload all the required documents which can prove your change in the birth certificate. You will get the list of documents on the website.
  • Click on submit.
  • The form will be verified by the authorities.
  • You will get an alert via SMS whether your form is accepted or not.
  • If accepted, you will get the new birth certificate within 30 days.

Offline Process:

  • First of all, you need to visit your municipal office near you.
  • You will get the form there for name or date of birth change.
  • Fill it up properly without any mistake.
  • Now, go to a notary.
  • Make an affidavit about your name or date of birth change.
  • State proper reasons for the change.
  • Now, after the affidavit is prepared, you need to again visit the municipal office with the affidavit, filled up form and all required documents. You will get the list of documents on the website.
  • Now submit the form attached with documents like the old birth certificate and address proof.
  • The form will be verified by the authorities.
  • You will get an alert via SMS whether your form is accepted or not.
  • If accepted, you will get the new birth certificate within 30 days.
